Emergency Treatment at Severe Pelvis Injuries: Physician Manual.
Gomel: Metal Polymer Research Institute of the National Academy of Sciences, 2003. - 296 pages.
A new research and practical trend in the surgery of severe pelvis injuries is
stated in the monograph.
Its constituents:
- new diagnostics - symptom groups of hidden injuries of bones and great pelvis vessels;
- new methods of conservative and surgical treatment of critically ill patients in shock;
- new surgeon's tactics - single-stage operations on several traumatic foci (including internal organs) according to principle "if it possible - all at once";
- new organization system - muitidisciplinary team emergency treatment at a big administrative region;
- new tools - a kit of 46 original special surgical instruments for pelvis;
- special personnel training for significant quality improvement of medical care to patients with severe pelvis trauma.
Surgeon's tactics at rendering emergency treatment to victims with severe pelvis injuries in the acute trauma period, especially in first minutes, hours, days after their admission to hospital is shown. Principles and methods of diagnostic and medical care at very severe pelvis injuries that are combined with damage of internal organs, great vessels in the retroperitoneal space, with multiple skeleton traumata, at open fractures, etc. are examined. The principle of surgeons' actions "if it possible- all at once" and the form of rendering multidisciplinary emergency medical care to such victims at a big administrative region via teams of broad trained surgeons are introduced.
Authorial developments in general pelvis anatomy, in classifications according to its instability, open fractures, complications, errors and the universal classification of injuries of pelvic ring, acetabulum and body outside pelvis at different mechanical traumata are described.
Developed by the author clinical and radial symptom groups of hidden injuries of great vessels in the retroperitoneal space, and also bones and junctures of pelvic ring in different places are illustrated. The proposed methods of pelvis fragments reduction by single-stage operations and skeletal extension, less traumatic new operative approaches, indications for conservative and surgical treatment of pelvic ring and acetabulum injuries are described.
Surgical armaments specially designed for pelvis are demonstrated. The results of treatment, complications, errors, fatal outcomes are analysed. World experience in rendering emergency medical care to critically ill patients with complex pelvis injuries is reflected. Ways of solving treatment problems of such patients in the big region and in the country are shown.
The monograph is addressed to orthopedists and traumatologists, surgeons, urologists, angiosurgeons, neurosurgeons, anaesthetists and experts in resuscitation, and specialists in radiation researches.
